  • DD-214
  • Health records (Military, Veterans Administration, Civilian)
  • Correspondence to/from the Veterans Administration.
  • Evidence of disability service connection. (For example, if you have hearing loss: Records confirming your exposure to loud noises. If you have PTSD, evidence of exposure to disturbing events. If you have prostate cancer or ischemic heart disease, proof that you served in Vietnam or another area where the military used agent orange.)
